ABSTRACT

We report a case of urachal actinomycosis. The patient presented with complaints of micturition pain and a left lower abdominal mass with pain. Magnetic resonance imaging(MRI) showed the mass extending from the dome of the bladder to just beneath the rectus muscle. Exploration revealed a hard mass in the urachal cord, which was attatched to a part of sigmoid colon and the dome of the urinary bladder and extended to the umbilicus. The mass, urinary bladder dome, sigmoid colon and urachal cord were resected, and histopathological revealed actinomycosis.

ABSTRACT

A clinical observation was made on the 236 patients who had undergone transurethral resection of bladder tumor(TUR) between Jan. 1990. and Feb. 1996. in the Department of Urology, Korea Cancer Center focusing on complications. The mean operating time was 59.0 minutes(n=400). The most common immediate complication was bleeding(1.8%), and the remainder was nonurologic fever(0.8%). The delayed complication was urethral stricture(1.0%) requiting visual urethrotomy. The immediate postoperative morbidity was 2.5% and risk factors for immediate morbidity after TUR were operating time longer than 60 minutes and tumor, invading the muscle or more deeply(p<0.05, by Chi-square test). The delayed morbidity was 1. 0% and the only risk factor for delayed morbidity after TUR was the number of ~IJR more than 2(p<0.05, by Chi-square test). Careful attention to surgical details and indications is needed to reduce the amount and significance of the postoperative morbidity.

ABSTRACT

We report a case of Castleman`s disease of the hyaline-vascular type which was located in retroperitoneum. A 20-year-old girl was admitted because of a history of postprandial epigastric discomfort. Computed tomography of the abdomen demonstrated a solid 7 x 7 cm mass between the abdominal aorta and the left kidney in the retroperitoneum. At surgery, a solid 9 x 8 x 6 cm tumor was resected. Pathologic diagnosis was Castleman`s disease of the hyaline-vascular type.

ABSTRACT

Total cystectomy was performed to the 40 patients with urinary bladder tumor who were found perivesical fat infiltration by computerized tomography. 25 patients were previously untreated, and 15 patients had undergone transurethral resection or biopsy of the tumor before computerized tomography. The histopathological stage was compared to the preoperative computerized tomography stage. In pathologic specimen, perivesical fat infiltration was found in 62.5% of the cases, whereas perivesical fat infiltration was not found in 37.5% of the cases. In untouched tumors or after transurethral resection computerized tomography accuracy demonstrated no significant difference.

ABSTRACT

Iliac artery aneurysm is rare disease. We report a 60-year-old man who was referred to our hospital because of right hydronephrosis and pelvic mass. A intravenous pyelogram, a computerized tomography scan and a magnetic resonance imaging revealed a pelvic sarcoma. We experienced a case of right internal iliac artery aneurysm masquerading as pelvic sarcoma.

ABSTRACT

We carried out an immunohistochemical study and DNA analysis of 14 surgical specimens of primary bladder carcinoma and paired sample of normal bladder mucosa to evaluate p53 over expression, point mutation by polymerase chain reaction single-strand conformation polymorphism analysis(PCR-SSCP). The immunohistochemical study demonstrated immunoreactivity for p53 protein in ten cases out of 14(71%) with monoclonal antibody (NCL-p53-DO-7, Novocastra Lab. UK). Mobility shifts were detected by PCR-SSCP in p53 gene exons 4-8 in six cases out of 14(42.9%). p53 gene mutations were observed in invasive cases(85.7%) and superficial cases (50%) by immunohistochemistry. Correlation was not observed between the presence of p53 gene mutations and the histological differentiation and invasiveness of the cancer. Considering frequent p53 alterations in invasive cases, it will be necessary to reevaluate p53 alterations in bladder cancer and the correlation to the prognosis in bladder cancer.

ABSTRACT

The synchronous presentation of two separate neoplasms is extremely unusual. In 1921, Graves and Temleton first discovered synchronous two neoplasms in the same kidney. However, almost all reported synchronous tumors arising in a same kidney have been renal cell carcinoma and transitional cell carcinoma. We report a case report of a synchronous renal cell carcinoma and adult Wilms' tumor in a same kidney in a 41-year-old woman which was managed by radical nephrectomy and combination chemotherapy.

ABSTRACT

Although genitourinary leiomyoma may arise in any anatomic structure containing smooth muscle, primary leiomyoma of the bladder is rare. This tumor is usually asymptomatic unless urinary tract function is affected. We experienced a case of leiomyoma of the bladder with back pain in a 42 year old woman.

ABSTRACT

The neutron beam has higher relative biological effectiveness (RBE) than conventional X or gamma ray because it is densely ionizing radiation which is presented by high linear energy transfer (LET). This physical and radiobiological characteristic plays an important role in killing of cancer cells in the state of biologically radio-resistant to the conventional radiation. The rationale of high LET radiation in the application to clinical radiotherapy is summarized as, high oxygen enhancement ratio (OER), less repair of cell damage, and less dependence of radio-sensitivity on cell cycle. Neutron therapy alone or combined with conventional radiotherapy was performed in 12 patients with stage C or D1 prostatic cancer from Mar. 1987 to Dec. 1989 in Korea Cancer Center Hospital. Local control rate at the time of 24 months after therapy was 67% (4/6) in stage C and 67% (4/6) in stage DI. Two-year actuarial survival rate after therapy was 82% in stage C and 67% in stage D1. The problem of neutron therapy was relatively high incidence of major complication rate, but it could be lessened by the accumulation of experience for neutron therapy. We think neutron therapy to be one of adequate treatment modalities for local control of stage C or Dl prostatic cancer.

ABSTRACT

An increasing attention has been called to such the problems as on drug resistance of bacteria, cross infection, or hospital infection in the recent antibiotic era. Urine from 131 patients supposed to have urinary tract infectionswere used for bacteriologic-culture and sensitivity determination for isolated icroorganisms in this study. Quantitative urine cultures in 96 cases were also performed and the following results were obtained; 1. Of a total of 171 bacterialstrains isolated by routine cultures of the urine, one hundred and thirty-three were gram-negative bacilli and remaining 41 (23.6%) were gram-positive cocci. Isolated microorganisms include Staphylococci, Klebsiella aerogenes, Pseudomonas aeruginosa, E. coli and Proteus species, in order of frequency. 2. The gram-negative bacilli showed considerable susceptibility to Kanamycin in a range of 58.1 to 90.4% and were less sensitive to Chloramphenicol and Tetracycline. Pseudomonas strains were susceptible only to Polymyxin B. and resistant to all the other antibiotics, Proteus and Klebsiella aeruginosa were resistant to all the antibiotics used in this experiment. 3. 78.3% of the bacterial strains isolatedwere resistant to more than four kinds of antibiotics. 4. Of 96 cases examined by serial quantitative urine cultures, 56 (58.3%) should more than 10 organisms per milliliter of urine and most of them were indwelling catheter.
